Day of your Appointment

Please arrive at our office 15 minutes before your scheduled appointment time to allow us to finish or fill out paperwork and stay on schedule with your appointment. Please bring the following items with you to your first appointment:

  • Identification such as Driver's License, State ID or Military ID

  • Dental Insurance Card (if applicable)

  • New Patient Forms

  • List of Any Medications Being Taken
